Social Worker
DEFINITION
Will provide initial medical social service assessment and subsequent delivery of direct social work services to the patient and family.
LEVEL OF RESPONSIBILITIES
Receives professional and personal guidance from Vice President of Operations, Director of Family Services, Clinical Manager and is accountable to the West Virginia Board of Social Work Examiners licensing board when applicable.
QUALIFICATIONS
Must have a BSW and/or MSW from a college/university accredited by the Council of Social Work Education. Must have a current West Virginia Social Work License. Must have a current driver’s license.
DUTIES and RESPONSIBILITES
- Provide direct medical social services:
- Completes initial psychosocial assessment within three (3) business days, but not to exceed five (5) calendar days from election.
- Explains purpose and goals of social services to the patient/family.
- Assesses patient status in areas including: family resources, support systems, status of caregiver, living arrangements, financial concerns, and patient/family ability to cope with disease process.
- Develops social service plan, using long and short term goals for intervention, involving the patient, family and other interested parties in the development of the plan.
- Implement plan of intervention, encouraging involvement of the patient and his family when appropriate, and empower them to make changes as needed.
- Provides information and referral services to the patient and family.
- Acts as a liaison between the patient/family and community resources.
- Attends meetings of and consults with Interdisciplinary Team regarding significant findings and/or changes in the patient’s condition.
- Synchronize initial psychosocial assessment by next day after assessment.
- Documents at time of visit and makes available social service intervention/documentation electronically, including frequency of visits, contacts with other agencies and duration of involvement.
- Complete Pre-Bereavement/Deaths Interdisciplinary Referral Form within 24 business hours.
- Participates in Interdisciplinary Team meetings.
- Responsible for assisting nursing with respite admissions: social worker will find available bed and arrange transportation to and from respite.
- Work in conjunction with facility social work when patient admitted into acute hospitalization.
- Work in conjunction with facility social worker in providing social services when patient resides in nursing facility.
- Work in conjunction with local DHHR and Adult Protective Services to ensure patient safety.
- Assess suicidal ideations and take necessary actions to assist patient and family. (See Suicide Assessment/Contract Policy)
- Work as a team with primary nurse to ensure that patient’s revoking or being discharged from hospice services have services that will be needed after d/c.
- Empower patient/family to make end of life decision, educates, and assist in completing advance directives. Obtain copies of advance directives for those who already have them.
- Work in conjunction with volunteer coordinator in volunteer placement.
- Complete any other duties as assigned by Executive Vice President, Director of Family Services and Patient Care Coordinator.
- Provides consultation and in-service education to agency personnel including involvement with volunteer training as needed.
- Assumes responsibility for social work services:
- Coordinates with other disciplines.
- Performs in accordance with Social Work Code of Ethics, standards and criteria of the profession as established by the laws of the State of West Virginia.
- Maintains a family and patient centered philosophy and approach, good public relations, and communication with the family and the agency members.
